1. Scope of Services

Andrea Nutt, LCSW (Verity, LLC) provides psychotherapy services to clients. Services are tailored to individual needs but do not substitute for medical advice or emergency care. If a client is non-compliant or presents with severe suicidal or homicidal ideation, the therapist may refer the client to a more appropriate provider.

2. Appointments and Attendance

  • Scheduling: All appointments must be booked in advance. Sessions will begin and end at the scheduled times, regardless of a client’s late arrival. Late arrivals will not extend the session time, and the full session fee will still apply.

  • Cancellations: Clients must provide at least 48 hours' notice to cancel or reschedule an appointment without penalty. Cancellations made within 48 hours of the scheduled session or missed appointments will incur a $135 charge.

3. Payments and Fees

  • Payment Policy: Copays and self-pay fees are due at the time of service. Accepted payment methods will be discussed during the intake process.

  • Late Fees: Late cancellations or no-shows will be charged $135, as outlined above.

4. Confidentiality and Privacy

All client information is handled in accordance with HIPAA regulations. Andrea Nutt, LCSW (Verity, LLC) utilizes HIPAA-compliant platforms, including SimplePractice and Google Workspace, to store and manage client data securely. Personal information will only be used for booking, billing, or treatment purposes and will not be shared without written consent, except as required by law.

5. Acknowledgment of Risks and Waiver

Psychotherapy involves personal exploration, which may include discussing difficult emotions or life events. While every effort will be made to provide a safe and supportive environment, clients acknowledge that results are not guaranteed. Clients must sign a waiver acknowledging these risks and confirming that psychotherapy is not a substitute for medical advice or emergency care.

6. Intellectual Property

All materials provided during therapy sessions, including handouts, exercises, or other resources, are the intellectual property of Andrea Nutt, LCSW (Verity, LLC). These materials are for personal use only and may not be reproduced, distributed, or used for commercial purposes without explicit written permission.

7. Updates to Terms of Service

Andrea Nutt, LCSW (Verity, LLC) reserves the right to update these Terms of Service at any time. Clients will be notified of changes via email or during their next scheduled session. Continued use of services after such updates constitutes agreement to the revised terms.

8. Dispute Resolution

Any disputes arising from or related to these services will be resolved through binding arbitration in accordance with the rules of the American Arbitration Association. By using these services, clients agree to waive the right to a jury trial or class action.

9. Limitation of Liability

Andrea Nutt, LCSW (Verity, LLC) will provide professional care and support but does not guarantee specific outcomes or improvements. Clients agree that the therapist is not liable for any decisions or actions taken based on the content of therapy sessions.

By scheduling and attending services with Andrea Nutt, LCSW (Verity, LLC), clients confirm that they have read, understood, and agreed to these Terms of Service.
